Spirometry in children primary health care

  Summary:
Spirometry is a fundamental test in evaluation of the pulmonary ventilation. Spirometry requires full cooperation of a child, and, thus, is applied on children above 5-6 of age. FEV1 (forced expiratory volume in the first second) is treated as the most important parameter of bronchoobstruction, since it is the most reproducible one. Spirometry tests indicate bronchoobstructive diseases, confirm bronchial hyperreactivity, which is responsible for bronchoobstruction during the respiratory infections caused by respiratory viruses and Mycoplasma pneumoniae. GOAL: The objective was to confirm that the spirometry was a powerful diagnostic tool for a paediatrician in the primary health care, for the purpose of diagnosing bronchoobstruction. MATERIALS AND METHOD: Evidenced paediatric examinations in the school dispensary in HC “Zvezdara“, Belgrade, during October, November and December 2005. RESULTS: Out of the total number of the 1105 first examinations of respiratory diseases, 113 included ausculatory signs of obstruction (asthmatics and children who had, at least one, obstruction). Thanks to spirometry, bronchoobstruction was confirmed in 25 patients/children, along with positive bronchodilatory test, whereby they had sparse ausculatory findings and had never experienced the obstruction before. CONCLUSION: Spirometry can be of great help for paediatricians in the primary health care, for the purpose of diagnosing bronchoobstruction, especially in cases of sparse ausculatory findings. Proper diagnosis leads to adequate treatment of a diseased child.
