Left Ventricular Myocard Hypertrophy at Patients with Hypertension
and Complex Heart Rhythm Irregularities

  Summary: The aim of this research was to detect and establish the frequency of complex heart rhythm irregularities in hypertensive patients with left ventricular myocardial hypertrophy. Method: The examinees were divided into 4 groups (3 groups with 35 examinees each – depending on the level of hypertension and 1 control group with 35 examinees as well) the average age being 50± 8, with 54 male and 86 female examinees. All of them were subjects of complete echocardiogram recording and Holter ECG monitoring. Arrhythmias grading was done according to Lown-Wolf classification. Results: Left ventricular myocardial hypertrophy was registered in 68 or 64.7% of 105 hypertensive patients. There were 45.59% multifocal ventricular extrasystoles during 24h in the examinees with left ventricular myocardial hypertrophy, ventricular extrasystoles in pair per 24h in 47.06% of patients, ventricular tachycardias per 24h in 40% of patients. The prevalence was several times higher than the prevalence in patients without the left ventricular myocardial hypertrophy (p<0.001). All mentioned heart rhythm irregularities are more frequent in the examinees of the concentric left ventricular myocardial hypertrophy subgroup than in those of the eccentric undilated left ventricular myocardial hypertrophy subgroup (p<0.05). The correlation between complex ventricular heart rhythm irregularities and echocardiographical parameters (diastolic intraventricular septum dimension, left ventricular posterior wall, left atrium dimensions, left ventricular mass index, right ventricle dimensions, isovolumetric relaxation time) was established by using logistical regression (BACKWARD). Such results can make implications on the therapy.
